What it measures
The free androgen index (FAI) is worked out from two results: total testosterone divided by SHBG, multiplied by 100. SHBG is the protein that binds testosterone, so FAI gives a rough guide to how much testosterone is free to act. It is a calculation, not a separate measurement.
Why people test it
- Used mainly in women when PCOS or excess hair growth is being looked into.
- Gives context when total testosterone looks normal but SHBG is low.
- Reported by many labs whenever testosterone and SHBG are tested together.
Typical range
Ranges vary by lab, method, age and sex, and the result depends on how each lab measures testosterone and SHBG. Your report shows the lab's own range. In men, many labs report calculated free testosterone instead, as FAI is less reliable.
Finger-prick or vein?
A vein sample is usually advised. Testosterone and SHBG are hormone tests, and finger-prick results can be raised by creams or gels on the fingers, especially hormone gels.
